CPPQ – Community Pharmacy Patient Questionnaire - Patient Survey
All pharmacies are required to conduct an annual community pharmacy patient questionnaire (CPPQ), also referred to as the Patient Satisfaction Questionnaire or Patient Survey. The questionnaire allows patients to provide valuable feedback to community pharmacies on the services they provide.
At Benjamin Chemist, we always try to provide you with the best possible service. We are asking NHS patients to complete a short and anonymous questionnaire about the service we provide.
You also have the option to download the forms and complete the questionnaire manually. Click here to download the Questionnaire in paper form. Once downloaded and completed, you can post the form to us or bring it in person.
We have Patient Questionnaire forms available in our pharmacy; please ask a member of the staff.
We will use the results of this survey to improve the services we provide at Benjamin Chemist.

HOW TO MAKE A COMPLAINT
